Client Compatibility Assessment

 

 

 

 

First, we conduct a client profile assessment which includes a cultural diversity background discussion, any relative medical conditions and personal safety review to determine if our services are needed and can be provided in the best interest of you, the client and your family.  

 

 

Secondly, after a sucessful assessment is completed, we will arrange a scheduled home visit based on the contractual agreement.  Visits normally take between 1-3 hours and will be implemented and conducted by a clearly identified representative.

All three tiers of the company checklist will be performed unless otherwise noted on the contract.

* We DO NOT require any of your family members personal identification documentation i.e. social security number or date of births.  All we require is an address, name, contact information and a non-binding contract. It's that easy!

 

Lastly,  The visits will be documented via checklist and provided to the contractually authorized representative. 

 

All Wellness Coordinators and Advisors are First Aid, CPR, AED certified and insured!
